Personal Care
At GCC, a qualified, well-trained staff member will be capable of providing a range of assistance to enable the member to accomplish tasks that one would normally do for themselves. Staff will allow enough time for the member to do as much as they can for themselves. When needed, staff will offer step-by-step instructions, and/or modeling (gesturing) of the task. Currently, GCC is unable to offer partial or physical assistance. Staff will always respect the rights of the member to direct how their personal care is provided.
Tasks may include: 1) Hygiene-Staff will offer verbal prompts (step by step instruction) for the member to complete the hygiene task. Staff may also provide modeling of the task; however, staff may not provide hands-on assistance at this time. 2) Bathing–Staff will offer verbal prompts (step by step instruction) for the member to complete the bathing task. Staff may also provide modeling (gestures) of the task; however, staff may not provide hands-on assistance at this time. 3) Eating-Staff may provide verbal prompts. Staff may offer partial or full physical assistance with meal prep. 4) Dressing-Staff may provide verbal prompts and modeling (gestures) with all aspects of dressing. Currently, GCC does not offer hands-on assistance. 5) Grooming-Staff will offer verbal prompts, modeling (gesturing) for shaving. Staff will offer verbal prompts, modeling (gesturing) with hair care. 6) Toileting-Staff will provide verbal prompts and modeling (gesturing). Hands-on assistance is not currently offered.
Additional Services
Verbal coaching with money management. This includes coaching on the bank account, budgeting, paying bills, saving money, grocery shopping, and paying for personal needs/wants. Verbal training is done on learning to make the correct change during transactions. These services are offered in the community (bank, grocery store, retail, and in the home).
Transportation (drive member in a vehicle) to stores to assist in personal care needs, such as purchasing hygiene products or grocery shopping. This is done in the community.
Staff will provide verbal prompts for the member to walk unassisted or with the help of adaptive equipment such as a walker or wheelchair. Partial or full physical assistance will be available if needed by the member to maneuver their adaptive equipment to ensure their health and safety while at home or out in the community.
Staff will provide verbal prompts, partial physical assistance, or full physical assistance-hands on with transfers and positioning (bathtub, toilet, bed, wheelchair) when a member is unable to accomplish the task independently. This will be done in the home and in the community.
Staff who is QMAP trained will give verbal prompts to the member in self-administering prescribed medications. This includes reminders for the members. The staff may provide partial physical assistance by opening a medication bottle or bubble pack and handing the medication to the member. This will be done in the home or community.
Staff will provide verbal prompts to members in any exercises of their choice. This will be done in the home or in a community setting such as a gym.
